Care management programs help your team track patients with ongoing conditions between visits.
Enrolling a patient
A clinician or care coordinator enrols a patient from the patient's chart. Enrolment records the program, the start date and the conditions being managed. A patient must give consent before enrolment, and that consent is recorded with the enrolment.
What enrolment adds
Once enrolled, the patient's chart gains a care plan with goals, scheduled touches and any assigned tasks. Time your team spends on qualifying activities can be logged against the plan, which supports accurate billing for care management services.
Keeping enrolment current
Review enrolled patients regularly. If a patient's condition resolves, or they are no longer a fit for the program, close the enrolment from the same screen it was opened on. A closed enrolment keeps its history so nothing about the patient's care is lost.
Who can enrol a patient
Enrolment is available to clinical and care coordination roles. If you do not see the option, your role may not include it, and your administrator can adjust that.
